Reporting COVID-19
COVID-19 requires all of us to be even more attentive to our health, regularly checking for symptoms associated with the virus.
For employees Continue to self-assess your health status and possible COVID symptoms each day before coming to a UCSC owned or leased facility. If you experience COVID symptoms, complete the symptom check questionnaire and report any positive test results to your manager or supervisor. You can also print a copy of the Employee Symptom Check Questionnaire in (English) or in Spanish (Español) Cuestionario de verificacion de simtomas del empleado.
Preventing the spread of COVID-19 is one of our highest priorities. UC Santa Cruz is responding to concerns from the campus community regarding COVID-19.
- Employees: If you have tested positive for COVID-19, or were exposed to someone who tested positive, please email your supervisor.
- Supervisors: Report confirmed COVID-19 cases or exposures to the COVID-19 Symptom Check Officer at check@ucsc.edu. Do not include additional health information.
- Students: If you’ve tested positive for COVID-19, please follow these instructions provided by the Student Health Center. If you have questions about what to do, you can also call the free nurse advice line at (831) 459-2591.
Santa Cruz County has instructions online for people about how to self-quarantine if they have been exposed to someone with COVID-19. People with questions about COVID-19 can call Santa Cruz County at 831-454-4242 between 8 a.m. and 6 p.m., Monday through Friday. Outside of these hours, people can call United Way of Santa Cruz County at 211 or text “covid19” to 211211.
